Why is standing so important for treating the most severely affected patients?

1. Hypertonia and spasticity can be significantly reduced by standing.
2. Daily standing can prevent contractions of the torso and of the lower extremities. In cases of strong spasticity in the plantar flexors, standing is the only way to maintain full dorsal flexion of the foot. The muscle group is too powerful for manual manipulation.
3. Mobilisation of the whole nervous system is performed more easily when standing.
4. Regular standing protects against osteoporosis and fracture
5. Standing upright appears to have a positive effect on vigilance. Patients become more awake.
6. Early and regular standing reduces fear of falling during later training in standing and walking.

11. Standing upright on your own feet again is an extremely positive experience. It is progress in therapy, which even patients with diminished vigilance can appreciate.
12. It offers family members, therapists and carers the opportunity to communicate with the patient at eye level.
